Payment Options
Paying the patient’s portion of the dental services can by done with Cash, Check, Visa, MasterCard, American Express or Discover Card. We also participate with CareCredit in offering no interest payment plans (based on a minimum charge and if paid in full in the time required, as defined in their plan).
Dental Insurance
Our office is in-network for Delta Dental Premier. We have many patients with other insurance carriers and their coverage and reimbursement generally depends on the out-of-network benefit for the particular plan.
While we are out-of-network for all other insurances, we are happy to work with your carrier to maximize your insurance benefit and submit all claims directly to them for reimbursement for your treatment.
For our patients with dental insurance, your portion of the payment is required at the time your service is provided. Your portion is determined generally by completing a pre-estimate of work to be done.
You are responsible for all portions of your bill not paid by your insurance plan for plans which we are out-of-network. This includes all Medicare / Medicaid plans. We are not opted into Medicare / Medicaid plans.
Pre-Estimates
For all our insured patients we recommend getting pre-estimates prior to your procedures so that you are aware of your portion of the procedure cost. Pre-estimates are just that, they are an estimate of what they will pay prior to your procedure. Dental work you have at other offices, including specialists, can reduce the actual reimbursement, as can annual maximums for individuals or families. We have no control over your insurance company, the plan that you have chosen, treatment outside of our office, or the plan maximums. Please educate yourself as much as you can with your plan details. We can provide you what details we have, but often we have access to less details than you, as the plan owner, might have.
For private pay and uninsured patients we offer two options:
An annual membership for Grand Avenue Dental (GAD) Affinity discount plan for discounts from 7% to 50% can be purchased. Ask your provider or the front desk for details.
A 3% courtesy adjustment for payment in full at the time of service via credit or debit card or a 5% courtesy adjustment for payment in full with cash or check.
– Note that no discount is given when using the CareCredit payment option.
