Payment Options 

We require payment of the patient portion of dental procedures be done at the time of service. Paying the patient portion of dental services can be done with Cash, Check, or Credit Card (Visa, MasterCard, American Express, and Discover are accepted).

Our office also participates with CareCredit.

Dental Insurance

Our office is an in-network provider for Delta Dental – Premier.

While we are out-of-network providers for all other dental insurances (including Delta Dental – PPO) we are happy to submit all dental claims directly to them for reimbursement for your treatment and to work with your carrier to maximize your insurance benefit.

We have many patients with insurance carriers that are out-of-network and their coverage and reimbursements through our office depend on the out-of-network benefit for their particular plan. We encourage patients to know their own specific insurance benefits when getting work done through our office. We will help with this task if we can.

For our patients with dental insurance, your portion of the payment is required at the time your service is provided. Your portion is determined generally by completing a pre-treatment estimate of work to be done. Your portion will be whatever is left over after your insurance pays their portion. Please note that insurance carriers tell us in all their correspondence that their pre-estimate is only that. It is an estimate, not a guarantee, of payment or coverage.

You are responsible for all portions of your bills not paid by your insurance plan for plans which we are out-of-network (all except Delta Dental – Premier). This includes all Medicare / Medicaid plans. We are not opted into Medicare / Medicaid plans.

Pre-Estimates

For all our insured patients we recommend getting pre-estimates prior to your procedures so that you are aware of your estimated portion of the procedure cost. Pre-estimates are just that, they are an estimate of what insurance will pay prior to your procedure. Dental work you have at other offices, including specialists, can reduce the actual reimbursement, as can annual maximums and deductibles for individuals or families. We have no control over your insurance company, the plan your company has offered and that you have chosen, treatment outside of our office, or the plan maximums. Please educate yourself as much as you can with your plan details. We will provide you with as much help as we can, but often we have access to fewer details than you, as the plan owner.

For private pay and uninsured patients we offer two discount options:

A 3% courtesy adjustment for payment in full at the time of service via credit or debit card or a 5% courtesy adjustment for payment in full via cash or check.

An annual membership in the Grand Avenue Dental (GAD) Affinity discount plan can be purchased for discounts of 50% on diagnostic and preventative care and 15% on restorative procedures. Ask the front desk for more details.
